In 2011, we played an important role in hosting the ‘African Footprint in Global Mental Health 2011’ World Mental Health Congress, and hosted our 17th Cape Town International Kite Festival, which celebrated ‘One Sky, One World — Mental Health for All”. Antipsychotic agents form the mainstay of treatment for many patients with serious mental illness. These agents are used in a range of psychiatric disorders, including the schizophrenia spectrum, bipolar disorder, and substanceinduced disorders. Local and international clinical practice guidelines for the treatment of schizophrenia1, 2 advocate antipsychotic monotherapy as the routine approach. Guidelines recommend avoiding antipsychotic polypharmacy (APP), which can be defined as co-prescription of more than one antipsychotic drug for a given patient.3 Exceptions to this may be when APP is required for short periods when switching agents or in treatment-resistant cases, when augmentation of clozapine with another antipsychotic agent may be considered, although supporting evidence for this remains weak.4, 5 Treatment guidelines recommend use of antipsychotics as part of treatment options in bipolar disorder, but do not advocate an APP approach.

My approach to therapy is pragmatic and eclectic with an emphasis on depth psychology and the importance of the unconscious. I pay particular attention to the power of habits both good and bad and I insist on the interdependence of physical health with mental or emotional health. I have Gestalt Therapy, Hypnotherapy and Cognitive Behaviour Therapy (CBT) in my repertoire and I use them where appropriate. I am steeped in the principles of Behaviour Modification and regard myself as a behavioural scientist. Finally I was credentialed as a Clinical Neuropsychologist by S.A.C.N.A. in 1987 and I practised as a Neuropsychologist for many years. Although I no longer practise Neuropsychology and I have let my registration lapse my knowledge of Neuropsychology ( such as it is) remains and it underpins my assessment for psychotherapy and my practice generally. I am a firm believer in lifelong learning and I spend at least two hours a week in the reading room of UCT Library perusing current journals. I also take time out of my busy week to attend lectures and workshop, devoting a minimum of 70 hours a year to this manner of keeping myself up to date. I have been doing this since I qualified as a psychologist in 1983 and the habit has become ingrained. Being an older therapist my knowledge and experience has been forged in the consulting room as well as from life generally.
